I realized early on in my career as a bedside nurse, that I wanted to be able to stand next to the people I care for in their vulnerable moments. I wanted to educate and affirm and support.

Hannah Lake-Rayburn, CNM, FNP is excited to provide whole-person healthcare in the Smyrna community. With a Bachelor of Science in Nursing from Goshen College and a Master of Science in Nursing with dual certifications as a Certified Nurse-Midwife (CNM) and Family Nurse Practitioner (FNP) from Emory University, Hannah brings a comprehensive approach to women’s health care.
Before joining Diana Health, Hannah spent nearly six years at Carle Foundation Hospital in central Illinois, working as part of a hospital-based midwifery group providing full-scope midwifery care to a diverse patient population, addressing both obstetric and gynecological needs. She was drawn to the care model at Diana Health as she firmly believes in the importance of patient-centered care, where the patients are in the driver’s seat, making informed choices about their health and well-being. She comes from a family legacy in midwifery—her mother was also a Nurse-Midwife, and her sister is a doula in northern Indiana.
She shares her home with her beloved long-haired black and tan dachshund, Nemo, who is spoiled and cherished. In her free time, she enjoys long-distance cycling and is passionate about baking, particularly sourdough, which has become “a delightful obsession” of hers.
