What is Chronic Care Management?

Chronic Care Management is the care coordination that takes place outside of your regular office visits. It was created to provide better care for patients with multiple (two or more) chronic conditions expected to last at least 12 months, and that place the patient at significant risk of functional decline, acute exacerbation, or death. Originally created by Medicare, this program is now covered by most private/commercial insurance providers as well.

What can I expect after I enroll?

Everyone who is enrolled in the program will receive:

  • A personalized care plan with a comprehensive, whole-health approach for managing their conditions
  • Monthly check-ins with a dedicated Care Manager to help with creating goals, discussing progress, answering questions, and coordinating additional care as needed
  • Periodic reviews of their care plan and medications, with revisions as needed
  • Prompt sharing of their medical records with other health providers, when needed for continuity of care

What is the cost?

Most insurance plans cover the cost of Chronic Care Management, however, usual cost-sharing requirements may apply. You may be responsible for any standard copay, coinsurance, and deductibles and may be billed for these up to once per month.

Collaborative Care Teams consisting of OB/GYNs, Certified Nurse Midwives, Mental Health Therapists, Dietitians, and Wellness Coaches, all work together to treat the whole you.

You’re always in the driver’s seat of your care experience. You set the agenda for your visits, specify your unique care preferences, and complete your comprehensive wellness plan, which we’ll use to guide your treatment.

Putting You in the Driver’s Seat (Educate + Empower)

  • You specify your preferences and share in the decision-making with your providers when developing treatment plans.
  • We offer a wide range of educational articles and tools to empower you with knowledge and ensure you feel comfortable and confident.
  • The Diana Health Client App allows you to view your lab results and medical history, and message your providers.
